Official Rules

All participants must be fourteen (14) years old and legal residents of one of the following Ohio counties at the time of entry: Athens, Coshocton, Crawford, Delaware, Fairfield, Fayette, Franklin, Guernsey, Hardin, Hocking, Knox, Licking, Madison, Marion, Morgan, Morrow, Noble, Perry, Pickaway, Pike, Ross, or Union.

TO ENTER

Step One: Nomination entries will be accepted beginning September 2, 2011 at 12:00 a.m. EST and ending October 2, 2011 at 11:59 p.m. EST (“Nomination Period”). To enter, got to NBC4i.com and search mascot or visit HelpingOhio.com during the Nomination Period. Entrants must register with Upickem with the following required information: (i) name, (ii) address, (iii) telephone number, (iv) email address (v) date of birth, and (vi) gender (vii) name of high school and (viii) name of the same high school’s mascot.

Any high school nominated must be an accredited high school and be located in one of the following Ohio counties at the time of entry: Athens, Coshocton, Crawford, Delaware, Fairfield, Fayette, Franklin, Guernsey, Hardin, Hocking, Knox, Licking, Madison, Marion, Morgan, Morrow, Noble, Perry, Pickaway, Pike, Ross, or Union.

Step Two: Voting entries will be accepted beginning October 4, 2011 at 12:00 a.m. and ending November 4, 2011 at 11:59 p.m. EST (“Voting Period”). The high school mascot with the highest score will be the First Place prize winner. The high school mascot with the second highest score will be the Second Place prize winner. The high school mascot with the third highest score will be the Third Place prize winner. In the event of a tie, the winner in each place will be drawn randomly. Participants who fail to provide any of the information required above will not be eligible to win and will not be notified by Sponsors. All entries become the property of WCMH and will not be returned or acknowledged. All timely and correct entries will be eligible for the Prize drawings.

Only one registered account per voter per day.

PRIZES

First Place: First place winner will win 1) their choice of QuizStyk video gear which includes: Camera HFM 300 or an equivalent camera, DVD player converted to screen, 9’ 6” Steel pole with Handgrips and bearings in the base. Total retail value: $2799. Or a $2,000 cash donation to their high school to be used in any manner that the school decides. If the school already has a QuizStyk they will be awarded the $2,000 cash donation.
2) $500 donation, in winning school’s name, to the HelpingOhio.com charity of the schools choice. Charities include: Flying Horse Farms www.flyinghorsefarms.org , American Red Cross www.columbus.redcross.org , YWCA www.ywcacolumbus.org , and Autism Puzzle’s charity of choice, Autism Society of Ohio www.autismohio.org . Charity donation is provided by Cecil & Geiser.

Second Place: Second place winner will receive a $1,000 donation to their high school to be used in any manner that the school decides. Donation is provided by Cecil & Geiser.

Third Place: Third place winner will receive a $500 donation to their school to be used in any manner that the school decides.

All details of all prizes will be determined by Sponsors in their discretion. Any portion of any prize not used by any winner is forfeit and no cash substitute will be offered or permitted. All prizes being offered are provided “as is” with no warranty or guarantee either express or implied by Sponsors. Merchandise prizes carry no warranty other than that offered by manufacturers. Winners assume sole responsibility for all costs associated with the prizes not explicitly outlined above, including but not limited to any and all taxes, fees and surcharges. All federal, state and local taxes (if any) are the sole responsibility of the winners. Other restrictions apply.

Prizes are not transferable, substitutable, or exchangeable for cash except that WCMH in its discretion may substitute a prize, or portion thereof, with a prize or portion of equal or greater value if it deems necessary. Any such changes will be announced. If any of the winners fail to meet any eligibility requirement herein, alternate winners will be chosen by random drawing to be held as soon as reasonably practicable after initial winners are determined to be ineligible. Alternate winners are subject to all eligibility requirements and restrictions of these Official Rules.

You must be 13 years of age or older at time of entry to be eligible for this contest. This contest is void where prohibited by law.

Winners will be notified by email. If a winner is unreachable after seven (7) days, or if that winner is unavailable for prize fulfillment, an alternate winner will be selected. If NBC4i.com cannot find an eligible winner for the prize, that prize will not be awarded.

Odds of winning depend on the number of eligible entries received.

Automated voting of any kind is not allowed. Detection of said automated voting will lead to votes being voided.

Only one registered account per voter per day. If multiple accounts are detected for a single voter, they will be disabled and votes disqualified.

NBC4i.com reserves the right to substitute any prize with another prize of equal or greater value.

NBC4i.com reserves the right to restrict the local winners to only persons whose primary residence is within a 60 mile radius of the affiliate's' office.

Employees of NBC4i.com, its respective parent, affiliates, subsidiaries, advertising sponsors and promotional agencies, and the immediate family members of each are not eligible.

Contest entrants agree to abide by the terms of these Official Rules and by the decisions of the contest commissioners, which are final on all matters pertaining to the contest.

Entrants further grant to NBC4i.com the right to use and publish their proper name and state online and in print, or any other media, in connection with the Contest.

NBC4i.com reserves the right to use any and all information related to the Contest, including information on contestants obtained through the contest, for marketing purposes or any other purpose, unless prohibited by law.

The Contest and all of the related pages, contents and code are copyright, UPICKEM, and/or NBC4i.com. Copying or unauthorized use of any copyrighted materials, trademarks or any other intellectual property without the express written consent of its owner is strictly prohibited.

Each winner, by acceptance of the prize, agrees to release all Sponsors, and their parent and subsidiary companies, their officers, directors, employees, agents, shareholders, affiliates, suppliers, distributors, and advertising agencies from all liability, claims, or actions of any kind whatsoever for injuries, damages, or losses to persons and property which may be sustained in connection with the receipt, ownership, or use of the prize.

UPICKEM and NBC4i.com are not responsible for Internet crashes or slowdowns caused by network congestion, viruses, sabotage, satellite failures, phone line failures, electrical outages, natural disasters or acts of man or God.

All results posted are unofficial until winners respond to e-mail notification.

Each winner is responsible for all federal, state and local taxes and will be required to complete an IRS form W-9. Sponsor reserves the right to withhold prizes until completed form W-9 is received.

This contest is not intended for gambling. If it is determined that a contestant is using the contest for gambling purposes they will be disqualified and reported to authorities.
